This is his first line of dialogue here, addressing Mitsuba. However, he draws out the syllables, and attaches "-kun" at the end. He almost never uses honorifics, and as far as I can recall, he never does it again with Mitsuba. For those that just need a quick refresher on the significance of the absence of honorifics, and the honorific "-kun":.
I figure this was because Tsukasa might've been trying to disguise himself as more of a benevolent wish-granting supernatural when he met Mitsuba, offering him a bit of respect. Tsukasa then drops the "-kun" immediately after the screenshot I showed you, switching to the simpler "Mitsuba".
I figure this shows the change in circumstances between them, and Tsukasa deciding to take things into his own hands. In Ch. I personally find this kind of fascinating. Here we have two occassions involving wish-granting, and those who subsequently paid the price for it.
